These handouts are general education. They describe how conditions and procedures usually work — not your specific case. Your provider’s instructions always take precedence. If something in a handout contradicts what you were told in the office, call us at (941) 324-2550.

New Patients

Preparing for your first visit

A little preparation makes the appointment far more productive. Please bring:

  • Photo identification and your insurance card
  • A written list of all medications, doses and supplements
  • Any prior imaging — CT, ultrasound, MRI — on disc, plus the reports
  • Recent lab work, especially PSA, creatinine and urinalysis results
  • Operative reports from any previous urologic surgery
  • The name and contact information of your primary care provider

If you were referred by another physician, ask their office to fax records ahead of time to (941) 251-3422. If your visit is for urinary symptoms, printing and completing the AUA Symptom Score or a voiding diary beforehand gives us far more to work with.

Arrive 15 minutes early for a new patient appointment so we can complete registration without cutting into your time with the provider.

Insurance & Billing

We accept most major insurance plans, including Medicare. Because plan networks change frequently, call our office with your plan name and member ID before your appointment and we will verify coverage and give you an estimated out-of-pocket cost.

Self-pay and transparent-pricing options are available for many office visits and procedures. Ask our staff for the current rate for the service you need — we will tell you the number before anything is scheduled.

Prescription Refills

The fastest way to get a refill is to ask your pharmacy to send an electronic request to our office. You may also call (941) 324-2550. Please allow two business days for routine refills, and request them during office hours rather than on a weekend.

Controlled substances cannot be refilled by phone message and may require an office visit.

Office Policies

Records, results and scheduling

Medical Records

Call the office to request a records release form. Completed forms can be faxed to (941) 251-3422. Records are typically released within a few business days.

Test Results

We contact you with results as soon as they are reviewed — normal or not. If you have not heard from us within a week of a test, please call rather than assume.

Cancellations

Please give at least 24 hours notice if you need to reschedule, so we can offer the slot to another patient waiting to be seen.

After Hours

For urgent problems outside office hours, call our main number for the answering service. For emergencies — severe pain, inability to urinate, heavy bleeding — call 911.

Forms & Paperwork

Disability, FMLA and pre-operative clearance forms can be dropped off or faxed to the office. Allow up to five business days for completion.

Privacy
